Purposes of Collection: We collect, hold and use any personal information provided to us in this form to process this AMP KiwiSaver Scheme Preferred Provider Application; administer the employer entity's (the 'Entity') employees' memberships of and investments in the AMP KiwiSaver Scheme; administer the AMP KiwiSaver Scheme, including the Scheme's compliance with any applicable laws or regulatory requirements; provide you and the Entity with materials, information and support services to help you and the Entity meet its obligations as an employer under the KiwiSaver Act; maintain relevant statistical records; provide you, the Entity and its employees with information about other products and services offered, managed or distributed by members of the AMP group of companies or other third parties; administer any products or services that you, the Entity or its employees subsequently sign up for; market research; conduct credit control and debt collection activities (including logging overdue debts owed to AMP with credit reference agencies); and notify the Entity of any changes relating to the AMP KiwiSaver Scheme Preferred Provider Agreement or the AMP KiwiSaver Scheme.
If we request personal information from you, and you do not provide it, or the information you provide is not accurate, we may not be able to process the Entity's application, or carry out any of the activities included in the Purposes of Collection paragraph above.
The personal information provided will be collected and held securely by AMP Services (NZ) Limited Level 21, AMP Centre, 29 Customs Street West, Auckland 1010. You can contact our Privacy Officer at AMP Services (NZ) Limited, PO Box 55, Auckland 1140 to access or correct any personal information. Acknowledgements (as per Preferred Provider Application form 0711)
I am an authorised signatory of the employer entity named (the 'Entity') and have the authority to complete this form on behalf of the Entity.
The Entity appoints the AMP KiwiSaver Scheme ('Scheme') as the chosen KiwiSaver Scheme for the Entity's employees who will be automatically enrolled into KiwiSaver and those employees who choose to opt in to KiwiSaver.
I authorise AMP Services (NZ) Limited ('AMP') to notify the Commissioner of Inland Revenue that the Scheme is the Entity's chosen KiwiSaver Scheme for the purposes of section 46 of the KiwiSaver Act 2006. The Entity's choice of Scheme shall be effective from the date the notice is accepted by the Commissioner of Inland Revenue, and may be terminated in accordance with the KiwiSaver Act 2006.
The Entity will comply with its obligations under the KiwiSaver Act 2006 and the KiwiSaver Regulations including distributing the AMP KiwiSaver Scheme Investment Statement to employees.
I understand that this appointment will continue until written or verbal revocation of the appointment is received by AMP and the Inland Revenue, or the appointment is revoked by AMP and the Entity notifies the Inland Revenue.
I acknowledge that if the Entity or a Scheme member has an adviser, AMP may pay the adviser a portion of the Administration Fee as set out in the Investment Statement.
AMP is the issuer and manager of the Scheme. The Trustee of the Scheme is The New Zealand Guardian Trust Company Limited (the 'Trustee'). I acknowledge that none of AMP, the Trustee, the underlying fund managers, any of their related companies, their directors, the Crown or any other person guarantees the Scheme, any investment in the Scheme, or any returns on an investment in the Scheme, or gives any warranty of reliability or accuracy, or accepts any responsibility arising in any way including from any error or omission. Past performance is not indicative of future performance and is not guaranteed by any party. Returns over different periods may differ.
I acknowledge that the Privacy Act 1993 provides me with the right to request access to and/or correct any of my personal information held by AMP (AMP in this context includes all members of the AMP Group of Companies and their subsidiaries, associated companies and agents) or the Trustee of the AMP KiwiSaver Scheme, and I authorise the use of any information supplied by me in connection with this application for the purposes listed in the Purposes of Collection paragraph and the disclosure of this information to third parties (including but not limited to the Entity's adviser, another intermediary or distributor, or a credit reference or debt collection agency) to the extent necessary for one or more of the Purposes of Collection, where otherwise required by law, or as otherwise authorised by me. The Entity's adviser or an adviser of the Entity's employees (if any) may have access to my personal information and AMP or the Trustee may disclose it to them for one or more of the Purposes of Collection.
